We are looking for color photos from all varieties of dialysis patients—younger, older, male, female, every ethnic group, people who are walkers, bike riders, and those more limited in their abilities. So, please join our Life Options Patient Scrapbook. You could be a star and inspire other patients!

Are you a kidney patient with a color photo you love—one that shows you doing something you enjoy? If you have a color snapshot you would like include in our Patient Scrapbook, send it to:

Also include a line that says, "I give Life Options permission to use my photo and the information about me I have provided for its website," and sign your name, date, and telephone number. If you prefer, we are happy to use only your first name to identify you. We will mail your photo back if you include your name and address.
If you have a digital photo you would like to share, you can e-mail it to us, in either .jpg or .tiff format. Include in your e-mail the same permission and other information requested for mail-in photos.
