Life Options Licensing Agreement

We invite you to view and download a copy of any Life Options booklet available on this website as a PDF file for your informational use. You may make and distribute copies of, or translate, the booklet requested, IF you email Life Options and and request a Licensing Agreement or download and email back to us the “Life Options Licensing Agreement” form which provides that:

  1. You reproduce and include in their entirety all copyright, trademark, and other proprietary rights and notices on every copy of the booklet.

  2. You do not modify the booklet in any manner, except as outlined below.

  3. You re-distribute the booklet FREE of any charges, including charges for reimbursement of copying or printing costs.

  4. You provide Life Options with copies of any translations into languages other than English.

You may identify yourself or your company as a provider of the booklet by adding your identifying information within a 3-inch-square box located on the lower right corner of the front or back cover of the booklet. The information that may be added is limited to the following:

  • Name
  • Logo
  • Mailing address
  • Telephone number
  • FAX number
  • E-mail address
  • Name of contact person
  • The words: “this copy compliments of:”

The downloaded booklet (or any other material downloaded from this website) may not be posted on any other website or otherwise made available for electronic distribution to third parties without our written permission.
